Disappearing items and duplication glitch

When I'm playing eggwars, there's a buf that sometimes happens where when I change the item I'm holding in my hand, the item I switch too turns into the item I was previously holding. This can be really annoying when I'm trying to fight someone and my sword randomly turns into a second bow or food. This is on bedrock edition.

Yes I have had the same issue and it killed me couple of times becuase i didn't catch i before I fixed it.
