Sentinel is actually always watching. Unlike other anti-cheats, Sentinel does not require to be activated with /report commands, but is constantly looking into everyone's behavior simultaneously. I can assure you that Sentinel has been banning an immense amount of cheaters since its release; probably two or three times as many as the staff team has banned in a couple of years with only a small group of false positives.

Keep in mind that the few cheaters you encounter are the ones that slipped through the anti-cheat. As Sentinel is constantly being improved, the minority of players who do still cheat will eventually be caught and punished accordingly. Our anti-cheat will always remain a work in progress, both to improve its detections and to compete with upcoming or updating clients.
