Ban/mute request templates

Hey!

I got an idea! This idea will most likely help mods and admins out alot. I'll not take the responsibility to make one. We need a required template that will say their IGN, the accused ones IGN, what he did, His name, proof, staff tags, time it happened in the +0 timezone and a non required explanation spot where you can explain what and where he did the hack and why you think he hacks.

I want the people who are gonna decide if people gets banned/muted or not if this is a good idea.

Not a required one ^^ the things I wrote was ideas. Think how much cleaner it would be if everyone used a template?

Wait, I'm comfused... What does that have to do with a ban request to get someone banned? If they aren't already banned, what is the need for a database then?

We can use the website to look up their name and get the right one.

Maybe make a unrequired one that is pinned to the front page on the "report a player" forum?
