Disconnected during a game

I got disconnected in a Tower Defence game while building a tower. The screen just said "Loading terrain..." and then it said "Disconnected." Does anyone know why?

I play on 1.12.2 Optifine

I also occasionally get stopped by Sentinel mid-jump in the lobbies or while running in game and that's a problem...

This might be unrelated, but I think it's worth mentioning. Sometimes, I just instantly crash in the middle of a game. I checked this issue with some friends. They said it might be a corrupt entity?

This is something I go through when I play tower defense with my friends. They said its some tower that crashes peoples games when they place it. I am running 1.12.2 and I crash every time they place it. I don't know what tower it is.

I mean I've seen all towers. It seems to occur randomly
