What if you fill in the youtube rank form and it is denied and later if you have more than 40000 subs and video's with cubecraft ip, and you ask again the YouTube rank form what will happend then?

No. If you fill it once. You can never fill it again. You must send a e-mail if you think the information you given is wrong.
