You cannot get them from chests. All custom made.

And how do you get them?

They were items cheated into the game through use of a hacked client. They could instantly kill people because they had level 29000 enchantments. A fix has been pushed to prevent players from getting these items, so you shouldn't see people cheating with them any more.
