Is a closed report a successful one? If an closed report appears does this mean it has been reviewed and accepted or something else?

No, your report needs to have resulted in the user being punished that report for it to be considered successful

So a closed report means the report has failed, correct?

Not necessarily failed since sometimes your report was closed as the reported user already got punished but yeah as I said your report needs to have resulted in the reported user being punished :))
