Spawner Mechanics

Do spawners spawn mobs when a player is in the chunk(s) near the spawner or is there a radius in which you have to stand for mobs to spawn? Also, are there any blocks mobs can't spawn on? I have tried everything from half blocks to carpet but they always seem to spawn, I even saw them spawn on ladders!

for them to spawn you need to be max 20 blocks away from them
